Fill in the gaps including the number of participants (roughly, you can update it later). Tick, if you request accommodation or trainings and mail this form to the organizers.

IF YOU REQUEST ACCOMMODATION go to the accommodation form (MS Word, 77 kb).
The accommodation order indicate number of persons, term and requested type of accommodation (school, youth hostel, hotel B, hotel A, own arrangements). Number of person for accommodation must correspond with number of participants in the main form. Details can be updated or changed. Don't hesitate to update it.

IF YOU REQUEST TRAININGS OR FRIEND MATCHES go to the training form (MS Word, 73 kb).
The training order. Write your order of training hours or friend matches. Specify how many hours. Price for one hour training or friend match is 35 EUR, to be paid by the ordering team. Organizers don't provide referees for friend matches. Trainings and friend matches are booked in the tournament arena (rink and goals included); the final schedule will be published on the Internet page.

Final participation confirmation

By the 31.03. the definite participation confirmation will be sent you by e-mail.

Provisional confirmation of your registration will be sent by return after your registration. You will be able to print it out. The registration is valid after participant fee payment only. Please e-mail or fax copy of the bank transfer. You can return to your registration form and update it.
